TRANSCRIPT ORDER FORM

STILL IN SCHOOL?

Contact your schools’ Guidance Department for assistance.
This Transcript Order Form is for people no longer enrolled in secondary school.

COMPLETING THE FORM

Complete the form as fully as possible.
Expect to present identification when you pick up your transcript.
Anyone else you send to pick it up must bring an authorization letter from you.
Transcripts are mailed upon request and receipt of fee.

ONCE YOU HAVE YOUR TRANSCRIPT

If you have concerns about the content of your transcript, contact the last high school you attended.
If you want records other than your transcript call 905 727-0022 ext. 2100.

The personal information collected on the order form will only be used to process your request as allowed under the authority of the Education Act. Call 905 727-0022 ext. 2015 for more information.
Information transferred over the internet could be viewed by an outside third party. You may prefer to order by calling 905 727-0022 ext 2100 or 2111.
Order forms are also available at all secondary schools.

Full Disclosure

Withdrawals and failures from Grade 9 and 10 courses are not recorded on the Ontario Student Transcript.

If a student withdraws from a Grade 11 or 12 course prior to five (5) instructional days after the issuance of the formal report card then the course will not be recorded on the transcript. If a student withdraws from a course more than five (5) days after the reporting period, a withdrawal (W) is entered in the credit column and the student’s mark at the time of withdrawal is recorded as a percentage in the achievement column of the transcript.

In addition, if a student repeats a course, each attempt and the mark will be recorded in the transcript with only one credit granted. The course with the lower mark will have an “R” entered in the credit column. Any questions concerning this regulation should be directed to a guidance counsellor.
